Transformation Manager

We are looking for a transformation leader ready to make a real impact.

This is a great opportunity to join a vital homelessness charity and play a key role in reshaping how people experiencing homelessness are supported across North London.

The successful candidate will lead an exciting programme of change, bringing housing, health, mental health, employment and voluntary sector partners together to create more joined-up, person-centred support and better outcomes.

You will be responsible for:

• Leading service transformation and continuous improvement
• Building and influencing strong multi-agency partnerships
• Improving referral pathways and support for people with complex needs
• Developing effective systems, processes and care planning
• Using data and insight to drive better outcomes
• Championing trauma-informed, person-centred practice

We are looking for someone with strong partnership, project management and communication skills, plus a good understanding of homelessness and complex needs. Experience across homelessness, housing, health or social care is highly desirable.
