💰 £21,802.65 – £22,465.84 per annum (pro rata to £26,366 – £27,168 FTE)

A leading Northampton-based college is seeking a dedicated and compassionate SEND Transitions Assistant to support students throughout their journey into education, employment, and adulthood. This is a rewarding opportunity to work closely with learners, families, educational settings, and external agencies to ensure every student receives the support they need to thrive.

The Role

As a SEND Transitions Assistant, you will coordinate and deliver personalised transition programmes tailored to the individual needs of students with SEND. You will act as a key point of contact for learners and their support networks, helping to create a smooth and positive transition into college and preparing students for their future progression.

Key Responsibilities

  • Deliver bespoke transition programmes for students with SEND, aligned to individual needs, EHCPs, and funding arrangements.
  • Work closely with students, parents/carers, schools, and Local Authorities to ensure successful transitions into college.
  • Support prospective learners in exploring course options and identifying the most suitable learning pathways.
  • Build positive relationships with students to promote independence, confidence, and personal development.
  • Liaise with internal college teams and external agencies to ensure appropriate support is in place.
  • Assist students in preparing for progression into further education, employment, or long-term support services.
  • Act as an advocate for learners, ensuring their voices are heard and their needs are met throughout their educational journey.

About You

We are looking for someone who:

  • Has experience supporting young people or adults with SEND.
  • Is passionate about helping learners achieve positive outcomes.
  • Has excellent communication and relationship-building skills.
  • Can work effectively with students, families, educational providers, and external agencies.
  • Is organised, proactive, and able to manage a varied workload.
  • Is committed to promoting personal, social, educational, and vocational development.

Essential Requirements

  • Full UK Driving Licence.
  • Access to your own vehicle.
  • Ability to travel locally as required.
